Our hospice and palliative care programs are available across 16 counties in North Texas, including : Collin, Cooke, Dallas, Denton, Ellis, Fannin, Grayson, Henderson, Hunt, Johnson, Kaufman, Rockwall, Tarrant, Van Zandt, and parts of Wise and Navarro Counties.

  • Essential Duties and Responsibilities :

  • Assess hospice patients to identify the physical, psycho social, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations.
  • Implement / develop / document the plan of care to ensure quality and continuity of care.
  • Applies concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
  • Provide clinical directions to the Hospice Aide / LVN to ensure quality and continuity of service provided.
  • Assure continuity of quality patient care delivered with appropriate documentation.
  • Assist in the training of new nurses and aides as needed.
  • Demonstrate commitment, professional growth and competency.
  • Promote Agency philosophy and administrative policies.
  • Perform admissions of new patients as the need arises.
  • Manage schedule with correct prioritization of patient need and make changes as appropriate.
  • Provide effective communication to patient / family, team members, and other health care professionals.
  • Other duties as assigned.
  • Qualifications, Skills, and Abilities :

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
  • Current Texas State license as a Registered Nurse.
  • Proof of current CPR.
  • One year of experience as a Registered Nurse in a clinical care setting required, hospice experience preferred.
  • Nursing skills as defined as generally accepted standards of practice.
  • Valid Texas Driver’s License required.
  • Must have Hepatitis consent / declination.
  • Must have proof of TB test.
  • Proof of valid Texas Automobile Liability Insurance required as listed individual.
  • Good interpersonal skills.
  • Must be able to read and write English.
  • Technical & Physical Requirements :

  • Prolonged standing and walking required.
  • Ability to lift up to 50 pounds and move patients.
  • Requires working under some stressful conditions to meet deadlines and patient needs.
  • Ability to make quick decisions and resource acquisition.
  • Ability to meet patient / family individualized psycho social needs.
  • Requires hand-eye coordination and manual dexterity.
  • Working Conditions :

  • Works in patients home in various conditions.
  • Possible exposure to blood and bodily fluids and infections diseases.
  • Must have the ability to work a flexible schedule.
  • Must have the ability to travel locally.
  • Some exposure to unpleasant weather.
  • Ideal Candidates Will Have :
