Job Summary and Qualifications

The Registered Nurse Case Manager is responsible for delivering patient-centered care by overseeing the plan of care throughout the patient's stay, managing their length of stay, and ensuring the effective use of resources. This role also involves coordinating a safe and appropriate discharge plan in collaboration with a multidisciplinary team. The RN Case Manager facilitates care transitions and promotes quality outcomes while balancing optimal patient care and resource utilization.

What qualifications you will need :

Education & Experience :

Associate Degree in Nursing or Nursing Diploma Required
Bachelor's Degree in Nursing Preferred
2+ years experience in case management or 3+ years experience in clinical nursing Required
InterQual experience Required

Or a combination of equivalent education and / or experience.

Licensure, Certifications, Training :

Currently licensed as a registered professional nurse in the state of practice, or possess an active compact license as per law and regulation. Applicants with a compact license from states outside Texas must :

Apply for a Texas RN license within 60 days from hire

Obtain current Texas RN license within 6 months of application for Texas RN license

Certification in Case Management Preferred
