Job description
Position Purpose:

Ensures that clients have an effective case plan, assess client progress toward timely achievement of results leading to self-sufficiency, and connects clients to resources to overcome barriers to achieving stable housing.

Essential Duties and Responsibilities:
  • Engages and builds relationships with families in an effort to understand the complex family dynamics that led to homelessness.
  • Coordinates the development of an individually tailored family case plan that delineates specific goals, objectives, and timeliness within 3 days of intake.
  • Provides task-centered counseling to enable clients to take specific action toward obtaining housing, employment, as well as county/state entitlement programs, while addressing the root cause of homelessness.
  • Initiates and completes comprehensive intake packet, as well as coordinates individual/family case plan meetings, ensuring that they will occur the first 3 days of admission and every 30 days thereafter.
  • Ensure clients who are assessed as appropriate for shelter diversion services are screened for diversion services within the first 7 days after intake and every 7 days thereafter for the initial thirty (30) days following placement.
  • Ensures F-SPDATs are completed with clients within 7 days of intake, and credit checks are completed within 14 days of intake.
  • Prepares monthly progress reports for Case Management and Employment Supervisor.
  • Ensures weekly and timely case note and data entry in HMIS, CATCH, and STEP within 24 hours or 1 business day per DHS Regulation.
  • Counsels residents regarding effective strategies to barriers that effect the implementation of their treatment plans and services.
  • Assists families in obtaining vital information, i.e. Social Security card, INS documentation and identification cards. Ensures that initial information upon admission, (i.e. signatures of legal guardian, consent forms, social security card, and birth certificate) is obtained and in the resident’s file.
  • Attends weekly case reviews with the Kennedy Short Term Family Housing team around case management and residential activities.
  • Assists Supervisor in coordinating and facilitating extension meetings 30 days after families’ arrival and then as needed.
  • Ensures that client files are maintained according to Agency, District of Columbia, Federal and COA regulations.
  • Assist with researching and locating stable housing leads for Kennedy Short Term Family Housing families, which may include leads in District of Columbia, Maryland, Virginia, or other jurisdictions.
  • Make appropriate referrals and assist clients with resources for moving.
  • Serves as a liaison between NCCF staff and other agencies providing services to the family.
  • Engages community resources for homeless residents, i.e. financial assistance, substance abuse treatment and support programs, trauma recovery groups, empowerment programs, etc.


Non-Essential Duties and Responsibilities:
  • Assists families in providing job search assistance and/or locate appropriate vocational training for residents.
  • Advocates on behalf of homeless families in local, state and national forums.
  • Participates in trainings that enhance knowledge on how to effectively work with diverse clients and families
  • Other duties as assigned.

Qualifications:
  • Associates Degree or 2 years of college required with at least two years experience as a counselor or case manager for a related population.
  • Exceptional teamwork orientation with good written/oral communication skills.
  • Exceptional interpersonal/conflict resolution skills.
  • Demonstration of cross-cultural competencies.
  • Must have a valid driver’s license.
  • An attitude compatible with the goals and purposes of the National Center for Children and Families.

Physical Demands/Work Environment:

While performing the responsibilities of the job, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers to handle or feel. The employee is occasionally required to stand, walk, reach with arms and hands, climb or balance, and stoop, kneel, crouch or crawl. Vision abilities required y this job require close vision working with a computer monitor. While performing the duties of this job, employee is occasionally exposed to moving mechanical parts and vehicles. The noise level in the work environment is usually quiet to moderate. While performing the responsibilities of the employee’s job, these work environment characteristics are representative of the environment the Human Resources Director will encounter. Salary: $60,000 - $65,000 Annually
