Job descriptionMaintain confidentiality of all client and office records in accordance with HIPAA guidelines and the Privacy Act PHI. Ensure effective communication and collaboration among the care team to provide cohesive and comprehensive care. Perform other duties as assigned to support the team and enhance patient care. Valid and active Registered Nurse license in good standing. At least one year of experience in a home health setting, or related area, with strong assessment skills. Excellent organizational and time management skills to handle multiple clients efficiently. Proficient in computer and internet-based applications, as well as office equipment. Competent working knowledge of client-based equipment. Must pass a criminal background check and sanction screening. Professional demeanor and appearance are necessary. Why Join Us? We are seeking a skilled Registered Nurse Case Manager to join our team.
This role is crucial in assessing patient needs, coordinating care, and ensuring effective communication among all parties involved in patient care.
As a Registered Nurse Case Manager, you will :
