Job Description
A member of the multi-disciplinary health care team responsible for the design, coordination and implementation of optimal diabetes care and education of patients with diabetes and their families in accordance with established ADA standards of care.
Responsibilities
Education : Provides comprehensive diabetes self-management training
- Considering age-specific physiological, developmental, and cultural / socioeconomic parameters, assesses needs of patients diagnosed with diabetes mellitus.
- Assists patients in determination of realistic and measurable goals or outcomes.
- Guides patients in identification of behaviors necessary for goal attainment within a designated time frame.
- Monitors progress made toward achievement of pre-determined goals.
- Provides diabetes self-management training in accordance with the nine content areas of the American Diabetes Association (ADA).
- Utilizes clinical practice recommendations and standards of care of the American Diabetes Association (ADA)
- May facilitate meetings of the Diabetes Educational Information Sessions or provide Diabetes related programs to the community.
- Provides in-services to health care providers on diabetes issues pertinent to patient care.
Communication : Maintains ongoing communication with other pertinent departments and health care providers of The Christ Hospital.

Qualifications
EDUCATION : Graduate of an accredited school of nursing. Bachelor’s Degree in Nursing or related field required. Master’s Degree in Nursing or related field preferred.
YEARS OF EXPERIENCE : Minimum of two years of medical-surgical nursing experience with patients diagnosed with diabetes mellitus required.
REQUIRED SKILLS AND KNOWLEDGE : Knowledge / use of adult learning principles required. Knowledge and expertise in diabetes mellitus and its management required. Excellent communication skills required. Strong verbal and written communication skills are required in addition to good interpersonal skills, problem solving, decision making, time management, and leadership skills. Computer skills in Microsoft Office, and EMR
LICENSES & CERTIFICATIONS :
Registered Nurse in the state of employment, Certification as a Certified Diabetes Care and Education Specialist (CDCES) preferred. Certification within 4 years of hire required if full time. Part-time and PRN requirement will be adjusted based on hours worked.
