Primary Purpose

As a Certified Diabetes Care Education Specialist, you will deliver advanced patient care for individuals living with diabetes. You will utilize established protocols and collaborative care plans to promote stability, prevent complications, and support optimal patient outcomes through patient-centered, value-driven approaches.

Minimum Specifications

Education

  • Graduation from an accredited nursing program or possess a bachelor's degree in Food and Nutrition.

Experience

  • Three years of clinical experience directly caring for patients with diabetes and related conditions.
  • Certification / Registration / Licensure

  • For RN : Current RN license from Texas Board of Nursing or Compact RN license.
  • For RD : Current Registered Dietitian certification and state licensure.
  • BLS certification from an approved provider is required.
  • Obtain Certified Diabetes Care and Education Specialist (CDCES) credential within six months of employment.
  • Skills or Special Abilities

  • Provide personalized diabetes self-management education and support tailored to individuals and support persons.
  • Demonstrate strong interpersonal skills and effective communication abilities with patients and colleagues.
  • Bilingual in Spanish is preferred.
  • Proficiency in basic computer skills, including knowledge of database software and electronic medical records.
  • Ability to deliver person-centered care in accordance with Texas Nurse Practice Act and established standards.
  • Responsibilities

  • Coordinate the referral process for community-based and inpatient Diabetes Self-Management Education (DSME).
  • Conduct comprehensive patient assessments and document relevant data and education plans in medical records.
  • Measure the effectiveness of DSME services through outcomes and continue to improve service delivery.
  • Collaboratively develop educational goals and individualized support plans for patients referred for DSME.
  • Work alongside a multidisciplinary diabetes team to enhance patient diabetes management.
  • Advocate for the needs and rights of patients, ensuring positive relationships with all stakeholders.
  • Contribute to the development of educational programs to increase capacity for optimal diabetes care.
  • Advocate for best practices in diabetes care in alignment with established standards.
  • Participate in community outreach activities to enhance public education and engagement.
  • For Dietitians : Assess nutritional risks and develop coaching strategies tailored to patients' needs.
  • Additional responsibilities may be assigned as necessary. Join us in making a real difference in the lives of those we serve!
