Certified Community Health Workers (CCHWs) are trusted frontline workers who bridge the gap between communities and health / social services. They empower individuals and communities by providing outreach, education, counseling, and advocacy. CCHWs engage marginalized populations, including those with mental illness, substance use disorders, and homelessness, using relational theory and stages of change concepts. Their efforts, aligned with MHMR Tarrant's mission, aim to reduce health disparities, and improve access to care. Known outcomes include increased access to behavioral health services, better health outcomes, enhanced communication between communities and health providers, and reduced reliance on emergency services, in line with CARF and CCBHC standards.
Minimum qualifications :
High School Diploma / GED
Substitutions : Associate Degree in health and human services field with no prior experience
Two (2) years ' Experience in Defined Experience : Community-based experience on projects working with serious mental illness, substance use disorders, HIV / STI prevention and testing, persons experiencing homelessness, HCV prevention and testing, and community advocacy, work / life experience.
G) License / Certifications : Valid Texas Driver's License, Certified Community Health Worker (within 6 months of hire date), Recovery Coach Certification (withing 60 days of hire date)
