This Home Health Physical Therapist opportunity is based in Waco, Texas, a vibrant mid-sized city in Central Texas known for its friendly community, rich history, and welcoming small-city feel with big-city amenities. Outdoor enthusiasts can enjoy Cameron Park, one of the largest municipal parks in Texas, with miles of trails, scenic overlooks, and river access for kayaking and paddleboarding. In this role, you will provide one-on-one physical therapy services to patients in their homes throughout Waco and the surrounding area. The position offers the opportunity to build meaningful relationships with patients and families, helping them regain function and independence in the comfort of their own environment. You will work with a variety of adult and geriatric patients who may be recovering from surgery, managing chronic conditions, or addressing mobility and balance issues. Typical responsibilities include completing initial and follow-up PT evaluations, developing individualized plans of care, providing therapeutic exercises and functional training, performing home safety and equipment assessments, and educating patients and caregivers on safe mobility, fall prevention, and long-term wellness strategies. You will collaborate closely with an interdisciplinary team that may include nurses, occupational therapists, speech therapists, and other healthcare professionals to ensure coordinated, high-quality care. A typical day involves traveling between patient homes within a defined service area of Waco and nearby communities, performing several visits per day depending on caseload and visit type. Visit lengths may vary, but you can expect a blend of evaluations, routine follow-up treatments, and reassessments. The home health model allows for a high degree of professional autonomy while still providing support from clinical leaders, coordinators, and office-based staff who assist with scheduling, referrals, and documentation support. Shifts are generally daytime hours on weekdays, with some flexibility based on patient needs and agency expectations. Weekend or on-call rotations may be available or required, depending on the specific role. The caseload is primarily adult and geriatric, and patient volume is maintained at a level that allows for thorough, individualized care and timely documentation. This position is ideal for a Physical Therapist who values independence, variety in their day, and the chance to see the direct impact of their work on patients’ daily lives.
