Job Description

Responsible :

• Transition inpatients to the clinic by seeing them within one week of hospital discharge and continuing the plan of care developed on the inpatient service (including follow-up of lab tests and procedures).
• Provide medical care to walk-in patients.
• Provide new patient medical evaluations and ordering of initial laboratory tests.
• Order pertinent diagnostic tests and procedures.
• Provide scheduled medical care to attending physician's patients unable to see their PCP (due to vacations, hospitalist month, etc.)
• Provide cross-coverage for other PA's as needed.
• Provide medication refills to patients throughout the clinic as needed
• Complete appropriate documentation of all evaluations, care, and services rendered in the medical record.
• Make appropriate referral for consultations.
• Communicate with referring or primary care physicians regarding patient care and treatment.
• Provide follow-up and health maintenance care including the appropriate adjustment of medications in accordance with established protocols or in response to specific instructions from the Attending Physician.
• Consults with colleagues, supervising physicians, specialists, and other health professionals as appropriate.
• Attend morning clinical conferences.
• Provide medical care for patients in the chronic disease collaborative.
• Provide medical care for patients in the Coumadin Clinic.
• Provide medical care for pre-operative patients needing medical clearance.
• Review results of all labs / studies ordered.
• Participate in Quality / Performance Improvement projects.
• Attend bi-monthly administrative meeting.
• Complete all related duties as assigned.

Minimum Qualifications

1. Current New York State registration as a Physician Assistant; or

2. Current limited permit issued by New York State to practice as a Physician Assistant.
