Job Summary
Performs professional public health nursing services focusing on prevention including reproductive health, cancer prevention programs, gap filling community health services, refugee health, tuberculosis screening and management, and immunization services for all ages. Individuals in this role practice primarily within the clinic setting. Performs professional public health nursing services focusing on patient education, provision of direct clinical services within a multi-disciplinary team setting, and care navigation to support continuity of care and improve individual health outcomes. This role assesses and appraises health needs of clients while administering tests and vaccines under provider orders, coordinates client care, maintains accurate health records, and performs administrative duties to support clinic function.

Demonstrate competency with direct patient care within Registered Nurse (RN) role and education appropriate to scope of practice as determined by licensure.
Provide client counseling on various health topics such as contraceptive counseling, vaccines, sexually transmitted infection (STI) risk reduction, chronic disease prevention, nutrition, and substance abuse.
Participate in programming to support access to cancer prevention education, facilitate access to cancer screening recommendations in alignment with evidence based best practice, facilitate patient follow through with referrals for additional screening or diagnostics at specialty providers, provide care coordination to support continuity of care.
Support individuals in making informed decisions about their health and lifestyle choices.
Collaborate with other healthcare professionals - physicians, nurse practitioners, nurses, medical assistants, care navigators, and other public health professionals - as part of an interdisciplinary team.
Conduct outreach and partnership building activities to strengthen community relationships in order to coordinate and deliver comprehensive health services to community members.
Review client vaccination history, provide client-centered education in alignment with Centers for Disease Control (CDC) recommendations and requirements, administer vaccines in accordance with evidence-based best practice recommendations in following provider or standing order, appropriate documentation and billing of vaccines within electronic health record (EHR).
Facilitate vaccine access for routine child and adult vaccines, immigration specific vaccines, pediatric catch-up vaccine schedules, and international travel vaccinations specific to patient itinerary and medical health history.
Manage clinic supplies and vaccine stock, and ensures compliance with vaccine, medication, and lab materials storage requirements and monitoring.
Plan and execute vaccination clinics and immunization programs within clinical and outreach settings to prevent the spread of communicable diseases.
Provide tuberculosis (TB) screening in clinic setting. Collaborates with provider to manage abnormal results and coordinates additional screening as directed. Provides patient education on active and latent TB and facilitates TB treatment for those eligible and interested. Conducts regular patient assessments and monitors TB treatment compliance.
Provide information and referrals for patients to maximize health, wellness, and connectedness to social and health programming within La Plata County Public Health Department and the larger community.
Make process improvements to clinic structure and flow. Including but not limited to process improvement within electronic health record (EHR) for medical documentation and medical billing, implementing policies and procedures within the clinical setting, and participating in continuous quality improvement activities.
Establish and maintain relationships with community members, leaders, and organizations to better understand their health needs and concerns. Organize and participate in community health fairs, workshops, and events to provide health information and services.
Maintain accurate and up-to-date records of patient interactions, interventions, and outcomes.
Complete accurate and timely data entry for services rendered through grant funding.
Prepare reports and documentation required for program evaluation, grant applications, and regulatory compliance.
Champion initiatives to improve community health by addressing health disparities, providing health education, and delivering essential healthcare services.
Demonstrate a high level of confidentiality in patient interactions and care. Provides high-quality, evidence-based, client-centered care to a diverse set of patients. Committed to providing culturally and linguistically appropriate services.
Maintain compliance with regulatory requirements, standards, and expectations for medical procedures, protected health information, and all other contract activities.

License to practice as a Registered Nurse in the State of Colorado.
