Description
- Be responsible for overseeing daily operations of case managers embedded in the PMG clinics, the Proactive Outreach Team, City Call and / or the Transition Call Team. The department provides case management services for at-risk patients, including patients with complex medical and psychosocial issues.
Bachelor's Degree in Nursing; graduation from an accredited school.
Experience in case management and care coordination in community, home services, hospital, managed care plan and / or medical group setting.

About the Team
Providence Clinical Network (PCN) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care. Collectively, our medical groups and affiliate practices are the third largest group in the country with over 11,000 providers, 900 clinics and 30,000 caregivers.
