Description

Astrana Health is seeking a dynamic RN clinical supervisor. This position involves overseeing ECM Health Risk Assessments, careplans implementation and sign of. They will manage a team of Lead Care Managers (LCMs) while providing clinical guidance and expertise for high-quality, patient-centered care. The role combines operational management, clinical oversight, and program development to ensure effective care coordination for individuals with complex medical and behavioral health needs. The RN Clinical supervisor will optimize the care management process, ensuring the implementation of care plans, staff training, and quality improvement efforts.

What You'll Do

Supervise, mentor, develop, and provide clinical oversight to a team of  ECM nonclinical / clinical LCMs, including onboarding and training staff

Provide clinical consultation on ECM Health Risk Assessments and review and sign off of patient care plans

Assist in ensuring smooth transitions of care from hospitals, behavioral health providers, and primary care physicians to prevent readmissions and ensure post-discharge follow-up

Participate in ongoing quality improvement efforts, such as case reviews, chart audits, and preventive care activities
