Job Summary
The Medical Liaison partners with a team of skilled professionals and families to provide services that are family-driven, youth-guided, trauma-informed, and culturally competent to help youth reach their full potential. The Medical Liaison supports and assists the Nurse Coordinator in the coordination and provision of health care to the youth served while providing direct supervision and care to the individuals they serve, assisting with their daily living skills.
This role is responsible for developing, implementing, monitoring, and reporting on treatment objectives and modalities that are utilized to support the youths' goals. The Medical Liaison will be involved in more complex problems / issues and may perform mentoring and training activities as assigned.
Essential Job Functions
- Assist and support the Nurse Coordinator in various tasks related to medical follow-up for patients which includes, but is not limited to : Scheduling and attending medical appointments. Documenting any changes and recommendations made by provider. Monitoring medical conditions of all individuals and notifying nurse of any changes. Managing prescription orders. Maintaining and completing documentation for all individuals, which includes scanning of documentation into the EMR. Attention to detail, adherence to protocols, and effective communication with the healthcare team to ensure the best possible care for individuals served. Pass and administer medication to individuals receiving services following established procedures.
- Inventory medical supplies, placing orders and restocking, as needed.
- Ensure the safety of all youth and families assigned to our care and apply appropriate emergency and trauma-informed crisis intervention principles and techniques, and Strategies for Crisis Intervention and Prevention (SCIP) approved de-escalation and intervention techniques up to and including physical restraints.
- Follow mandated reporting protocols.
- Support youth and family interactions within the family and local community.
- Mentor staff on site to provide day to day support for individuals on site, including getting individuals prepared to attend program, providing direct supervision, ensuring daily documentation is completed, and ensuring oversite of the physical plant is a safe, sanitary environment.
- Provide supervision and care to the individuals they serve, assisting with their daily living skills according to established program standards and follow all applicable safety and mandated reporting protocols.
- Develop, implement, monitor, and report on treatment objectives and modalities that are utilized to support the individuals' goals. Assess the effectiveness of individual care plans (e.g., behavior, safety, treatment, and routine support plan).
- Ensure program requirements are met by compliance with Office of People with Developmental Disabilities (OPWDD) regulations and Home and Community Based Services (HCBS).
- Support the staff development of the DSPI and DSPII through mentoring and training.
- Serve as shift charge, document behaviors targeted for intervention, and provide transportation to appointments or home time as directed.

SPECIAL CONSIDERATIONS
19.46 Minimum pay rate, $27.75 Maximum pay rate, based on experience.
