Job Summary

Join our dedicated team as a Community Nurse Practitioner Specialist, where you will provide essential screening, preventive primary care, and medical services to members in comfortable environments like their homes, community spaces, nursing facilities, and 'pop-up' clinics. Your contributions will help enhance member health outcomes while supporting our mission for quality, patient-centered, and cost-effective care.

Essential Job Duties

  • Deliver general medical care and care coordination across diverse patient populations including adults, women, pediatrics, and geriatrics.
  • Conduct comprehensive evaluations through history-taking and physical exams to identify care gaps and preventive needs.
  • Address both chronic and acute clinical complaints promptly and effectively, assessing medical urgency accurately.
  • Establish reasonable medical diagnoses and seek specialty consultations as necessary.
  • Order and perform relevant diagnostic laboratory and radiology tests judiciously within resource constraints.
  • Recognize when to escalate care to a supervising physician, ensuring optimal patient safety.
  • Create and implement personalized medical plans of care tailored to patient needs.
  • Coordinate post-discharge care to minimize hospital readmissions and emergency room visits.
  • Conduct face-to-face visits in various settings including in-home, skilled nursing facilities, and community locations.
  • Utilize telehealth methods for face-to-face consultations where appropriate.
  • Participate actively in community-based 'pop-up clinics' to enhance healthcare accessibility.
  • Travel up to 120 miles daily to deliver care, with potential extended travel for special projects as needed.
  • Maintain multi-state licensure as required for business purposes.
  • Collaborate with fellow nurse practitioners to streamline best practices.
  • Engage in regional meetings and initiatives.
  • Possess the ability to prescribe medications and perform procedures in compliance with regulations.
  • Ensure timely documentation in electronic medical records (EMR).
  • Occasionally lift equipment weighing up to 50 lbs and navigate stairs.
  • Engage in medical practice under the direction and supervision of licensed physicians following state laws.
  • Local travel required according to state and contractual obligations.

Required Qualifications

  • At least 1 year of experience as a nurse practitioner or a relevant combination of education and experience.
  • Active national certification from AANP or ANCC.
  • Current, unrestricted Family Nurse Practitioner (FNP) license.
  • Active, unrestricted DEA license to prescribe medications.
  • Current Basic Life Support (BLS) certification.
  • Valid driver's license, reliable transportation, and adequate insurance for travel.
  • Ability to adapt to diverse populations and varied work environments.
  • Self-motivated, independent worker capable of managing time effectively.
  • Strong communication skills, both written and verbal.
  • Proficiency in Microsoft Office and EMR systems.
  • Preferred Qualifications

  • Experience as a registered nurse or nurse practitioner in home health, community health, or public health settings.
  • Background in managing chronic health conditions within home health.
  • Familiarity with underserved populations facing healthcare access challenges.
  • Skills in immunization and point-of-care testing.
  • Bilingual abilities are a plus.

    Pay Range : $79,607.91 - $172,483.80 ANNUAL. Actual compensation may vary based on geographic location, work experience, education, and skill level.
