Clinical Liaison
Job Summary:
We are seeking a highly skilled and compassionate Clinical Liaison to join our team at Advanced Healthcare Center in Toledo, OH. As a Clinical Liaison, you will play a critical role in building relationships with healthcare professionals and facilitating the transition of patients from acute care settings to our skilled nursing facility. If you are a dedicated and patient-focused individual with excellent communication skills, we encourage you to apply for this exciting opportunity.
Responsibilities:
Develop and maintain relationships with healthcare professionals, including hospital case managers, discharge planners, and physicians to facilitate the transfer of patients to our skilled nursing facility
Conduct interviews with patients and family members to assess their needs and preferences, and develop individualized care plans to meet those needs
Collaborate with the interdisciplinary team to ensure seamless transition of patients and effective communication with healthcare providers
Participate in care conferences and other meetings to ensure that patients' needs are being met and to identify opportunities for improvement
Provide education and support to patients and families on our services and benefits
Identify and address any barriers to patient care and work with the team to resolve issues
Maintain accurate and timely records of patient interactions and communication with healthcare professionals
