Job Summary : We are seeking a compassionate and dedicated Clinical Social Worker to join our esteemed healthcare team. As a Clinical Social Worker, you will play a crucial role in providing direct social work services to our patients and their families, ensuring they receive the best care and support possible. Key Responsibilities :

Direct Patient Care : Provide clinical social work services to patients and families, including individual and group counseling on the impact of illness and hospitalization.

Patient Advocacy : Champion patient rights and ensure confidentiality, advocating for patient needs within the healthcare system.

Treatment Planning : Assess patient and family needs to develop and implement effective treatment plans, adjusting as necessary based on patient progress.

Interdisciplinary Collaboration : Work closely with other healthcare professionals to ensure comprehensive and continuous care for patients.

Resource Education : Educate patients and families about available community resources and services to support their healthcare journey.

Communication Facilitation : Serve as a liaison between the patient, family, and healthcare team, facilitating clear and effective communication.

Record Maintenance : Maintain detailed and accurate records of patient interactions and treatments.

Team Participation : Actively participate in team meetings and case conferences to coordinate care and discuss patient progress.

Qualifications :

Education : Master's degree in Social Work from an accredited institution.

Experience : At least 1-2 years of clinical social work experience, preferably with acute and chronically ill patients in a hospital setting.

Licensure : Must be licensed or license-eligible as an Independent Clinical Social Worker (LICSW) or LGSW in Washington, .

Skills : Strong commitment to patient advocacy, excellent communication and interpersonal skills, and a thorough understanding of the healthcare system.
