The Opportunity

This position will collaborate with other professionals to evaluate pediatric patients medical or physical condition and to assess client needs as part of the interdisciplinary team. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual pediatric patients and the medical community.

Job Summary

Primary Responsibilities :

• Provide psychosocial assessments of patients and families with social emotional interpersonal and / or environmental issues. Formulate biopsychosocial assessment disposition and treatment plans.
• Collaborate with other professionals to evaluate patients medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitor evaluate and record client progress according to measurable goals described in the treatment and care plan.
