CCBHC CORE DUTIES & RESPONSIBILITIES :

  • Demonstrate mentorship and support for clinical staff, by becoming subject matter expert in evidenced based practices and population specific interventions. Includes facilitation of meaningful learning experiences for in the F&CS internship program.
  • Serve as a back-up supervisor of the team as needed. When Clinical Supervisor is unavailable, provide designated coverage for leading scheduled team meetings, staffing complex client cases, and assisting with crisis interventions.
  • Assist Clinical Supervisor with workflow management, which directly affect team performance and client outcomes. This may include collaborating with Clinical Supervisor when reviewing data, implementing PQI initiatives, providing individual or group supervision, and optimizing processes for service delivery.
  • Assist Clinical Supervisor with ensuring staff are well trained and adhere to all necessary service provisions and clinical requirements defined in CCBHC standards.
  • Assume a lead role in collaboration with community partners, including housing providers, hospital providers, law enforcement, inpatient hospital staff, jail staff, and community shelters.
  • Provide comprehensive onboarding to new staff, including orientation to on-site job functions, assistance with initial training and credentialing, access to essential workplace resources, and ongoing follow-up to ensure employee competence.
  • Responsible for carrying a reduced caseload and meeting service expectations for clients, based on program specific requirements.
  • Attend and participate in daily organizational meetings and treatment team meetings with multi-disciplinary team to utilize a team based care approach when working with clients.
  • Document client progress to maintain a permanent record of client activity according to established methods and procedures.
  • Demonstrate understanding of various best-practice treatment models to work with individuals with Severe Mental Illness, Co-occurring disorders, and trauma.
  • Understand the wrap-around focus of CCBHC services, assisting clients in multiple life areas through multi-disciplinary teamwork and community referral.
  • Obtain and maintain professional licenses and certifications required for the position through attending required trainings, tracking CEU’s, and submitting renewals in a timely manner.
  • Be familiar with CCBHC standards, understand the history, philosophy, structure and purpose of CCBHC and demonstrate fidelity to the model.
  • Effectively complete and submit all clinical documentation with established agency timelines.
  • Utilize assessment tools to complete diagnostic evaluation of clients and communicate this information to the interdisciplinary treatment team. This includes clinical diagnosis, as well as screening for serious mental illness, substance use disorders, history of trauma, and co-occurring disorders.
  • Conduct initial and ongoing clinical assessments, using established evidenced based assessment instruments. Information gathered from clinical assessments will be shared with other members of the interdisciplinary treatment team, for the purpose of developing an individualized, strength based plan for care
  • Work collaboratively with client, other treatment providers, and family members or natural supports (when appropriate) to develop a clear plan for treatment. Treatment plans are created using a collaborative client driven process and will include measurable outcomes, treatment goals, documented progress, necessary interventions and after-care plans (when appropriate).
  • Use evidenced based practices approved by F&CS, to provide therapy services to individuals consistent with their identified needs based on their biopsychosocial history and ongoing clinical assessments. Therapeutic services are individualized based on each client’s unique clinical needs, personal strengths, cultural background, motivation to change, and access to resources.
  • Deliver therapeutic interventions that are consistent and congruent with the chosen model of treatment and theory of change, developmentally and culturally appropriate, sensitive to individual needs of the client, and grounded in evidenced based modalities. Interventions are connected to the treatment plans goals and are modified when necessary, based on client needs and therapeutic progress.
  • Effectively utilize F&CS clinical pathways to assess for suicide or violence to others. Collaborate with other members of the treatment team to ensure appropriate clinical steps are followed to safeguard the well-being of the client and / or others. This may include, but is not limited to, risk assessment, collaborative safety planning, and crisis response.
  • Appropriately utilize clinical supervision, case consultation, live supervision and in-service training in the service of managing cases and increasing clinical knowledge and skills. Seek supervisory consultation and take appropriate action when ethical and legal dilemmas emerge. Practice competence and internal credentialing within defined scope of practice.

  • POPULATION SERVED : ACT Teams provide intensive, community-based services to individuals with severe mental illness. Individuals served by ACT Teams often have complex mental health needs, including acute psychosis, histories of complex trauma, and co-occurring substance use disorders. As such, these individuals experience very high behavioral health and physical health risks, which have not been successfully addressed by traditional forms of outpatient treatment. The primary function of ACT Teams is to help reduce unnecessary hospitalizations and incarcerations, by providing individuals the resources essential to thrive and maintain independence in their natural environment.

    POSITION SPECIFIC DUTIES & RESPONSIBILITIES :

  • Maintain a strong working knowledge of the core ACT principles and ensure clinical practices reflect a high standard of model fidelity, including Chapter 55 regulations (if applicable).
  • Collaborate with Clinical Supervisor on implementation of PQI quality improvement plans to enhance service delivery and ensure compliance with CCBHC standards. Assist in communicating organizational and programmatic changes to the team through clinical supervision and monthly department meetings.
  • Assist in leading daily organizational meetings and schedule staff assignments based on clinical needs; co-facilitate treatment team meetings with multi-disciplinary team, utilizing a team approach in working with ACT clients.
  • Ensure new ACT referrals are screened in a timely manner and coordinate with Clinical Supervisor to facilitate admission when appropriate. Upon admission, complete all necessary intake paperwork, assessments, and develop initial treatment plan.
  • Provide direct client care for a 70% caseload, which includes the following services :
  • Medication delivery and support to clients in the community.
  • Promote healthy lifestyles, including general wellness education and information specific to chronic medical conditions.
  • Have good working knowledge of community resources for basic needs such as food, shelter, and clothing. Refer and link ACT clients as needed.
  • Provide clients with therapy and / or psychosocial rehabilitation services to help support successful functioning in the community, including education in the areas of ADL’s, socialization, education, employment, and family functioning.
  • Assist interdisciplinary team with the completion of ACT initial comprehensive assessments and ongoing clinical assessments as indicated.
  • Provide assessment for high-risk clients and when appropriate follow agency-established protocols for ensuring client safety.
  • Support ACT team members and provide clinical guidance, when treating individuals with high demands or complex treatment needs. When necessary, provide direct intervention to address escalated situations.
  • Be available to respond to crises in the community, utilizing agency protocols for crisis intervention. May include completion of LMHP evaluations and coordination with law enforcement to ensure client safety.
  • Be available to work non-traditional hours, which may include evenings, weekends, Holidays, and on-call overnight (varies by team). This may also include staffing crisis situations during non-scheduled shifts.
  • Utilize agency issued cell phone during work hours for ACT work. Staff are expected to be responsive to cell calls and agency approved electronic communication, when scheduled to work. This may include overnight for on-call shifts (varies by team).
  • Have reliable transportation and be willing to utilize personal vehicle for community-based work, which may include transporting clients when appropriate.
  • Demonstrate comfort and flexibility while working with clients in difficult life circumstances, which may include individuals, who are homeless or living in substandard conditions.
  • Develop strong working relationships with leadership of internal FCS programs, to ensure effective communication and optimal workflow. Example programs include : FCS Pharmacy, Crisis Care Center, COPES, and Intake department.
  • Coordinate with community partners to ensure continuity of care and advocate for client needs. Example of community partners include : housing providers, inpatient hospital staff, shelter staff, and law enforcement.
  • QUALIFICATIONS :

  • A Master's degree in Social Work or its equivalent from an accredited school.
  • Knowledge and experience in individual, group and family therapy.
  • Knowledge and experience in conducting mental health assessment, diagnosis and treatment planning.
  • Must demonstrate a clinical family systems orientation and ability to work with diverse populations with multiplicity of problems.
  • It is required that all Therapists who are eligible for licensing by the State of Oklahoma be licensed annually and all other Therapists seek appropriate supervision and meet standards that make them eligible for licensure.
  • Must possess a valid Oklahoma Driver License and satisfactory driving record and use personal automobile to travel to locations other than primary office.

    Family & Children’s Services works to heal hurting and abused children, strengthen families, and provide hope and a path to recovery for those battling mental illness and addiction. You will be working in an innovative and dynamic environment using a new transformative model of care Certified Community Behavioral Health Clinic (CCBHC). This model is characterized by innovative, team-based approach and whole person care for adults and children with a range of complex mental health and substance use challenges.

    Utilizing multi-disciplinary teams, staff work collaboratively to provide a coordinated effort to enhance client recovery. Robust CCBHC services often result in better client outcomes and quality of care due to :
