Empower patients by engaging, educating, and coaching them to navigate health systems
Connect patients with essential resources to address social drivers of health
Collaborate with the care team to develop and execute patient care plans
Required Qualifications
High school diploma or GED required; relevant lived experience is welcome
Prior healthcare experience preferred, especially in geriatric care
Proficient with Microsoft Office Suite and electronic medical records (EMR)
Reside in the state of Florida
Demonstrates professionalism and reliability
