Community Health Worker

Qualification and Education Requirements

High School Diploma or equivalent
Completed TDH approved CHW Certification course
Current Driver's License & Reliable Transportation
Excellent observation, communication and relationship building skills
Proven experience in healthcare or community outreach role
Ability to work independently and collaboratively within a team
Experience with EMR software is a plus

Essential Duties

Assists with recruitment and enrolment or participants in home setting

Provides emotional support to patient and their families, fostering open communication and support

Assists staff in identification and connection to resources for patients and families

Develops relationships with community organizations and facilities to secure resources and helps to complete appropriate applications

Assists in coordinating home health services, medical equipment and things related to medication management

Maintains accurate and up to date documentation of up-to-date documentation of patient interactions and interventions

Ensure compliance with all relevant healthcare regulations and standard

Performs other duties as required.
