The Program for Assertive Community Treatment (PACT) is an intensive, community-based multidisciplinary treatment team that provides person-centered care to a shared caseload of adults whose needs are not able to be met through traditional outpatient services. The team provides case management, individual and group therapy, initial and ongoing assessments; employment and housing assistance, family support and education, substance use services, and other services and supports critical to an individual's ability to live successfully in the community.
The PACT Team participates in rotating weekend and holiday shifts to provide coverage for our clients during non-business days. If a weekend day or holiday shift is worked by a PACT employee, the PACT employee will be able to take a different day off that same week.
The Mental Health Clinician II will provides services that include but are not limited to engagement, supportive listening, psychoeducation, skill building, crisis assessment, prevention and intervention, resource education, develop and implement treatment plans, coordinate, and assist client with obtaining services while measuring client progress toward established goals and objectives.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
1. Maintain client records and all other required documentation in a timely and confidential manner in accordance with agency policies and procedures.
2. When required by the outpatient program, MHC IIs who hold a credential (i.e., L-AAC, LMHCA, LMFTA, LICSWA) eligible to be a Mental Health Professionals will provide initial intake assessments for persons seeking services at FBH to determine individual meets medical necessity and eligibility for FBH outpatient services.
3. 4. Involves comprehensive assessments, preliminary diagnoses, and treatment recommendations for both youth and adults.
5. Involves completing Consent to Release Confidential Information with the client and guardians, when appropriate, for any current and past behavioral health treatment, involved allied service provider, and natural supports.
6. Intakes should be completed concurrently with the client.
7. Accurately determine the program assignment for individuals who are enrolled.
8. Conduct risk assessments, develop and implement safety plans, and provide crisis intervention and stabilization services as needed, including facilitating access to inpatient facilities for behavioral health care.
9. Represent the agency and its services to the community in a positive and professional manner.
10. Participate in required staff meetings, including regular supervision and daily clinical staff meetings.
11. Participate in any required training, consultation calls, and onsite visits or review when required by contract.
12. Participate in weekend and holiday coverage shifts as needed.
13. Accept delegation and follow through with specific expanded assignments and responsibilities within FBH system such as quality improvement teams, task groups, committees, consultations, mentoring or training responsibilities.
14. Establish and maintain collaborative working relationships with; clinical, administrative, support staff, community partners, clients, and family / natural supports.
15. Contribute to out-reached based, consumer-centered treatment, rehabilitation, and support. 80% of services must be provided on an outreach basis in the community.
16. Identify and include the client's support network and applicable involved community partners in care coordination.
17. Develop and implement care plans, measurable objectives and obtainable goals, in collaboration with the client, evaluating client progress and adjusting as needed.
18. Assist clients in identifying and accessing resources and collaborate with community resources on behalf of clients.
19. Assist clients in the development of healthy coping mechanisms in individual or group settings.
20. Perform FBH charting requirements, correspondence, Workday and other paperwork in a complete and timely manner.
21. Operate as a member of multi-disciplinary team and ensure services are provided in the community and clinical setting in required by contract.
22. Other duties as assigned.

PAY RANGE ( Hourly / Non-Exempt) :
Mental Health Clinician II - No Licensure : $31.84 - $38.79
Mental Health Clinician III - Licensure : $33.70 - $41.06
Mental Health Clinician II - No Licensure : $31.84 per hour.
Mental Health Clinician III - Licensure : $33.70 per hour.
