Job descriptionSocial Work Experience Engage members in discussions about their health. Review member service records and conduct comprehensive assessments. Identify urgent situations and escalate when necessary. Develop Plans / Recommendations for Member Care and Service Work with members to develop healthcare goals. Identify support systems and community resources. Collaborate with members to create solutions and refer them to appropriate programs. Assist With / Coordinate Plans / Recommendations to Improve Member Health Coordinate access to health-related programs and services. Follow up with members to monitor progress and make necessary changes to plans. Educate Members / Caregivers in Self-Management and Plan Benefits Provide education on preventive healthcare measures and plan benefits. Educate members on disease-specific signs and symptoms. Promote a culture of health and wellness. Use Technology to Obtain and Document Health-Related Information Research member-related health information. Use computer applications to document and communicate member information. Navigate multiple applications to obtain and document information.

Requirements :
- High school graduate with 2+ years of experience in a community health setting in a healthcare role.
- Bilingual - English and Spanish
- Experience working remotely
- Willingness to work in MST
Preferred : CNA / LPN / CMA Certification
Essential Job Functions : Conduct Assessments of Member Healthcare Needs
Pay : $16-18 / hr
