WHAT YOU'LL DO

  • Grow meaningful referral networks - Build and maintain relationships with physicians, hospitalists, discharge planners, case managers, SNFs, acute rehab centers, and hospital systems.
  • Be the face of our brand - Represent our home health and hospice services at community events, health fairs, and professional meetings.
  • Educate and advocate - Lead in-service presentations that help referral sources understand our full scope of care, eligibility criteria, and care transition capabilities.
  • Drive census growth - Execute strategic territory plans that consistently grow referral volume across both service lines.
  • Collaborate cross-functionally - Work hand-in-hand with our admissions coordinators and clinical team to ensure seamless referral-to-admission experiences.

WHAT WE'RE LOOKING FOR

  • 2+ years of sales, marketing, or business development experience in home health, hospice, or a related healthcare setting
  • An established referral network - you know the physicians, case managers, and SNF contacts in your territory
  • Proven track record of meeting and exceeding census growth targets in a competitive market
  • Strong communicator - you can translate clinical concepts for non-clinical audiences effortlessly
  • Self-starter mentality with excellent time management across a large, diverse territory
  • Familiarity with Medicare home health and hospice eligibility preferred
  • Valid driver's license and reliable transportation required
