Eliot's Developmental Disabilities division is currently seeking an experienced candidate for Community Support Case Manager. The ideal person will have the ability to advocate for community supports and services on behalf of the individuals.
Eliot's Developmental Disabilities Division is a CARF accredited, individual-centered continuum of services that provides innovative and high quality programming to adults with developmental disabilities. The programs offer support and opportunities for individuals to enrich their lives, obtain housing and employment, develop personal skills and competencies, and enjoy a full range of community activities while developing a network of caring relationships.
Responsibilities :
- Assist the Director in completing assessments and Individual Service Plans(ISP) / Plans of Care(POC) for assigned cases in a timely manner and as indicated by DDS and MRC policies and standards.
- Ensure individuals are involved in identifying and developing their own goals.
- Write and maintain all documentation, related to individuals' treatment plans, in a clear and well-organized manner.
- Ensure all written materials are written in measurable, observable, behavioral, objective and clinical terms.
- Document daily progress notes for all individuals in accordance with Eliot Policy and DDS / MRC documentation and billing requirements.
- Progress notes shall be completed at the time of contact with an individual or on behalf of an individual and reflect specific treatment interventions as indicated on POC or ISP.
- Complete monthly progress notes by the 5th of the following month and submit to Director for review and sign off.
- Submit monthly service delivery billing documentation to the Director by the 30th of each month.
- Develop and maintain a positive and professional working relationship with individuals, families and providers. This includes maintaining a set schedule with individuals, timely communication and follow up on issues and requests and being on time for appointments, meetings and activities.
- Coordinate service delivery for cases assigned i.e., case conferences, Section 8 Housing, SSI and advocate for the consumer with outside agencies and providers.
- Provide service linkages to other providers as necessary. Identify and schedule appointments, for / with individuals, as needed or required.
- Ensure all relevant information is both communicated and documented to ensure quality of service.

Hourly Rate : 19.50
