Description
Job Summary :
The Outreach specialist will participate in outreach events, connect with community partners, and educate the community about the Cal-works home visitation program. The outreach specialist will work collaboratively with the NRC to promote services and connect families to resources. The outreach specialist will carry a small caseload of 4 to 6 cases and assist with monthly group connections.
Essential Functions :
- Assist in linking families to appropriate services and resources for support.
- Provide presentations about the Cal-Works Home Visitation program and participate in outreach events.
- Work collaboratively with the NRCs and support outreach events
- Must be able to establish, develop, and maintain collaborative partnerships and networking resources to support the needs of the families.
- Must be able to work independently and be self-motivated
- Carry a caseload of 4-6 families
- Must be trained in the Parent's As Teachers Curriculum
- Must be able to plan, provide, and document personal visits focused on parent-child interaction, development-centered parenting, and family well-being.
- Make regularly scheduled home visits on all families assigned
- Promote, coordinate, and assist in monthly group connections
- Must be able to establish relationships with families and children
- Position requires data entry utilizing the Apricot and Visit Tracker database, answering phones, and various computer activities.
- Maintain and submit required family documentation and data in a timely manner
- Administer assessments and screenings
- Participate in reflective supervision monthly
- Be familiar with EPU policies and procedures and maintain proper confidentiality.
- Maintain, manage, and process client records and information in a confidential manner that meets HIPAA and other applicable confidentiality regulations.
- Position requires some local travel.
- Keep an accurate mileage log when utilizing your own vehicle for home visits, meetings, and / or training.
- EPU facility hours are Monday - Friday, 8 : 30 AM - 5 : 00 PM, with some exceptions based on family's needs. May be required to work evenings and weekends.
- Must perform other duties as they are assigned by the Site Supervisor, Home Visitation Manager, Director of Parent Services or President / Chief Executive Officer.
Requirements
Knowledge, Skill, and Experience :
Education : Minimum of an Associate's Degree in Social Work, Child Development, or other related field. Bachelor's Degree is strongly encouraged.
Skills : Must have strong communication skills, written and verbal, planning / organizing, information gathering, identifying solutions, summarization, listening comprehension, goal setting, multi-tasking, teamwork, and
collaboration, initiative, and able to make independent decisions within guidelines and code of ethics. Must be self-motivated.

Physical Demands :
Typical Working Condition : Position works from the EPU facility. The position requires the ability to sit and work on a computer for extensive periods of time. Position requires fingering (fine dexterity) and repetitive hand and wrist motions.
Equipment Used : Computers, printers, telephones, and fax machines. It may require the operating of a vehicle for local travel.
Essential Physical Tasks : Must be able to sit for long periods of time, working with a keyboard or computer screen.

Salary Description
23.00 - $24.00 per hour DOE
