The Case Manager will assess client needs, develop care plans, coordinate services, and ensure compliance with funding and agency requirements. This role requires experience with HMIS (Homeless Management Information System), strong collaboration skills, and knowledge of community resources. ESSENTIAL DUTIES AND RESPONSIBILITIES : Client Assessment & Case ManagementConduct comprehensive client assessments, including cognitive, physical, social, economic, and emotional functionality, to determine service eligibility.Perform home visits to interview and observe clients in their home environment.Develop, implement, review, and update individualized care plans in collaboration with clients, families, and support networks.Monitor client progress, evaluate service effectiveness, and ensure adherence to federal, state, and agency policies regarding eligibility, reassessment, and monitoring.Provide ongoing case management support, including for clients receiving financial assistance.Service Coordination & Resource NavigationFacilitate access to essential services such as transportation, housing, home care, food assistance, and social programs.Assist clients with benefits eligibility review, enrollment, and recertification for programs including Medicare, Medicaid, LIHEAP, SNAP, and housing assistance.Coordinate services with external providers to secure necessary support for clients.Compliance, Reporting & DocumentationMaintain and update all client records in HMIS and Salesforce in a timely and accurate manner.Ensure all documentation, including consents, assessments, client goals, progress notes, and discharge / care transition summaries, is compliant with CARF accreditation standards and funding requirements.Track and report financial assistance disbursed through grants or contracts.Assist in collecting data and preparing reports for grant proposals and program evaluation.Community Outreach & Professional DevelopmentConduct outreach to referral sources and community partners to promote case management services and other services at JF&CS.Participate in staff meetings, supervision sessions, and required trainings.Qualifications & Skills : Bachelor's degree in social work, human services, or a related field (Master's degree preferred).Minimum of 2 years of case management experience, preferably working with low-income or at-risk populations.Experience with HMIS (Homeless Management Information System) is required.Strong knowledge of public benefits, housing assistance programs, and social services.Proficiency with Salesforce (or ability to learn quickly).Ability to conduct home visits and work independently in the community.Excellent organizational, communication, and problem-solving skills.Understanding of CARF regulations and experience working within grant-funded programs is a plus.Contract Terms : This is a contract position up to 35 hours / week with specific deliverables tied to grant funding through June 2025 with the possibility of extension based on funding availability.Must have reliable transportation for home visits and community outreach. Knowledge of local resources, a plus.Candidates who are selected for employment must successfully complete a pre-employment background screening as well as a pre-employment drug screening. Employment will be contingent upon the satisfactory completion of both the background check and the drug screening.
