Complete prior authorizations when needed. Provides accurate and appropriate documentation of patient / family services. Facilitates Hospice / home health / palliative referral process, serves as a resource to review and consult available patient information, to determine hospice care needs and criteria; coordinates and communicates with Hospice agency and referral sources to promote care coordination and a timely admission process. Are you passionate about providing support and care to individuals and their families during challenging times? Sangre de Cristo Community Care is seeking a dedicated and empathetic individual to join our team as an Admissions Coordinator.
Position Overview
As an Admissions Coordinator, you will play a crucial role in facilitating the admissions process for patients seeking hospice, home health, or palliative care. You will be the first point of contact for families, guiding them through the admission procedures with compassion and understanding. Your role is pivotal in ensuring a smooth transition for patients into our care.

- Competitive Salary - $20-$25 per hour
- Mileage reimbursement of sixty-two and a half cents per mile
