As a Counselor , you will play a key role in providing vital support to our residents, helping them make informed care decisions and successfully transition to higher levels of care. Primary Responsibilities :

  • Resident Support & Assessment : Conduct wellness orientation, psychosocial assessments, and assist with transitions to higher care levels.
  • Family & Staff Collaboration : Provide guidance to families, caregivers, and interdisciplinary teams on the psychosocial and emotional needs of residents.
  • Mental Health Services : Conduct assessments for mental health, anxiety, depression, cognitive function, and provide brief counseling or referrals when necessary.
  • Crisis Management & Support : Offer intervention and support for residents experiencing emotional distress, loss, or other challenging life events.
  • Programming & Education : Lead support groups and wellness-related presentations for residents and staff.
  • Consultation & Coordination : Work with health and mental health professionals to ensure comprehensive care for residents.
  • Community Outreach : Foster connections with local community resources and provide referrals when appropriate.
  • Resident Advocacy : Uphold the dignity and autonomy of residents, promoting self-advocacy and involvement in their care decisions.

Key Qualifications :

  • Education : Master's Degree in Counseling, Social Work, Psychology, or a related field (Gerontology certificate a plus). LPL or LCSW preferred
  • Experience : 5+ years of experience in counseling or social services, particularly in long-term care or with older adults, or an equivalent combination of education and experience.
  • Skills : Strong interpersonal and communication skills, proficiency with Microsoft Office, and the ability to work independently and as part of a team.
  • Knowledge : Understanding of elderly care needs, Alzheimer's disease, and the importance of preserving resident dignity and self-determination.
  • Licensing : Must maintain valid state professional licensure and a commitment to ongoing education and professional development.
