JOB DESCRIPTION
Position Title : Care Coordinator III- Young Adult Court
Employment Classification : Full Time / Non-Exempt
Reports to : Clinical Supervisor

Program Summary :
The Young Adult Court Program (YAC) works in collaboration with University of California-Irvine, Orangewood Foundation, Probation, the District Attorney's Office, Defense Attorney's Office, and The Orange County Superior Court. The YAC program provides engagement, assessment, clinical case management and resource coordination support to Transitional Age Young Men (ages 18-25) as part of a collaborative research study to reduce recidivism.
Position Summary :
The Young Adult Court Care Coordinator III plays a vital role in supporting participants through a trauma-informed, person-centered approach. This role involves providing ongoing case management, utilizing motivational interviewing techniques, and working closely with a multidisciplinary team to support participants' progress. The Case Manager III will also be responsible for documenting participant progress, coordinating services, and ensuring that participants have access to the resources they need to achieve their goals. The program offers a structured support system through four stages : Engagement, Accountability, Stability & Sustainability, and Attainment.

Requirements
Major Areas of Responsibility :
- Provide personalized case management to participants, helping them set and achieve individualized goals while addressing their basic needs and building essential life skills.
- Facilitate client orientations, workshops, and prosocial activities, engaging participants in meaningful and supportive activities.
- Utilize trauma-informed practices and motivational interviewing to create a supportive environment and effectively engage participants throughout the program.
- Collaborate with a multidisciplinary team to provide comprehensive and cohesive support, ensuring that participants receive the resources they need.
- Monitor and document participant progress, providing regular updates and reports to the team, including court-related documentation.
- Assist participants in accessing and navigating community resources, ensuring they have the support needed to achieve their goals.
- Identify opportunities and challenges, providing coaching and mentoring to help participants overcome barriers and succeed in the program.
- Provide 24 / 7 phone support to participants in crisis, offering assistance with de-escalation and ensuring their safety and well-being
