Job description : The Care Management Services Director is responsible for the strategic planning, organization, and management of patient care facilitation and transitions within a hospital setting. The role oversees case management, social work, utilization management, and post-acute services coordination to ensure optimal patient flow and quality care.
Key Responsibilities Develop, implement, and oversee hospital-wide case management programs to enhance patient transitions, care coordination, and overall patient flow.
Lead and manage a multidisciplinary team, ensuring alignment with hospital-wide patient experience goals.
Improve case management processes, ensuring compliance with regulatory requirements and best practices.
Optimize resource utilization through effective discharge planning, utilization review, and post-acute care coordination.
Collaborate with physicians, nurses, and healthcare providers to streamline care transitions and minimize patient readmissions.
Drive key performance metrics, focusing on patient outcomes, cost management, and operational efficiency.
Ensure strong financial stewardship by implementing cost-effective patient management strategies.
Maintain and promote effective communication channels between leadership, clinical staff, and external stakeholders.
Qualifications : Must-Haves :
Current RN License (or eligibility to obtain).
5+ years of case management experience in a hospital or healthcare setting.
Strong communication and customer relations skills.
Nice-To-Haves :
Masters Degree in Healthcare Administration, Nursing, or related field.
3-5 years in a supervisory or leadership role.
Formal training in case management best practices.
