Please confirm this VIA A COMMENT If your bid is accepted, you must submit the credentialing documents 14 days after acceptance or your bid runs the risk of cancellation. All credentialing documents are located in the "other documents" tab. An active license IS mandatory for every bid. Only list the license number in the field, not an IMLC letter number. Provider must be contracted with the agency prior to placing a bid. ***PLEASE NOTE: There will be no OT rate for this assignment request*** Assignment Request message for vendors: **IMPORTANT NOTICE** All providers are required to be credentialed and enrolled prior to the start-date so please refer to the Other Details tab on this Assignment Request for the Facility Checklist.
