Job Description
Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease. Provides nutritional assessment, education and counseling to individuals and / or groups. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility.
Job Responsibility
- Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease.
- Assesses nutritional needs and considers patients' cultural, educational, and learning issues.
- Provides individualized nutrition education to patients, their families, and / or significant others based on their needs assessment.
- Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility.
- Communicates patient information to referring providers and appropriate staff in a timely manner.
- Documents nutrition information discussed, written materials provided, and evaluation of patient's receptiveness and comprehension of the education provided.

Job Qualification
Preferred Qualifications
Registered Dietitian and Certified Diabetes Care and Education Specialist certificate preferred.
