Overview
The Care Partner Universal Worker works collaboratively as part of a household team to create an environment that encourages resident involvement and personal growth. Under the supervision of a registered nurse, the primary responsibilities of this position include delivery of exceptional nursing care in a household environment, assistance with the delivery of an overall dining experience and the collaborative evaluation, and implementation of programming that will mirror normal life activities of residents (to include the upkeep and management of personal living space) mental and emotionally engage their interest and assist them in recapturing the value of normal community life.
Essential Function s
- Creates and maintains an atmosphere of warmth, person-centered care, personal interest and positive emphasis as well as a calm environment throughout the household.
- Reviews the resident's plan of care with the charge nurse with any new assignment, changes or when in need of additional explanation or teaching.
- Provides personal hygiene (bath, shower, shampoo, assistance with elimination needs, dressing, or other personal care to include shaving and fingernail filing) as directed by the plan of care and reports any change of conditions to charge nurse.
- Performs safe transfers and assists with ambulation safety using assistive devices such as cane, walker, wheelchair, and mechanical chairs.
- Completes all required documentation as well as documentation needed for billing.
- Assists residents with meals, including food preparation as needed, monitor dining room, assist in serving, assists with menu selection and submit menus to dining services.
- Completes basic household chores to include making bed, laundry, trash removal, washing dishes, checking refrigerator for spoiled food, etc.
- Prepares residents for appointments and assists with transportation within the community and accompanies them to outside appointments as needed.
- Utilizes standard precautions at all times and reports a potential exposure or hazardous condition to charge nurse.
- Responds to “call for aid” as directed.
- Completes all required education programs and at least 8 hours of dementia education annually and demonstrates skills specific to providing care to residents affected by dementia.
- Performs other duties as assigned.
Education and Experience
Hours
16 hours per week, 11 : 00pm-7 : 30am (Week 1 : Sunday / Wednesday, Week 2 : Wednesday / Saturday)
19.00 - $23.00 per hour (based on years of experience) + shift differential (Weekday nights : $1.50, Weekend nights : $2.00)
