We also have a satellite clinic offering Intensive Outpatient Programs in the West Valley region. Case Manager Position Summary :

The Case Manager works under the guidance of the Clinical Director and Clinical Lead, while mentoring case management staff. The Case Manager collaborates with other healthcare team members to coordinate and facilitate the effective delivery of quality services to Calvary patients. The Case Manager is knowledgeable about and responsible for treatment plans, utilization review, discharge planning, psychoeducation groups, and case management.
