We are now hiring a professional discharge planner that is ready to hit the ground running! Come and join our professional and rapidly growing team. The qualified candidate will have to find the patient's placement on a timely and effective way
Responsibilities :
Overseeing and completing all of the discharge processes such as making an appointment with doctors and community mental centers, communicate with family, guardian / Proxy, DCF, ALF and ILF, document thoroughly all the process that takes place with the patients, find placements based on patient's criteria and level of care. We offer an excellent compensation package and additional incentives.
Requirements :
- Current Florida Driver's license
- 3 to 5 years prior experience, preferably in a psychiatric hospital setting
- Computer literacy to include e-mail, Microsoft Office and various medical billing systems.
- Be able to maintain a positive and organized work environment.
- Practice excellent interpersonal skills and maintain professionalism at all times.
- Possess excellent written and verbal communication skills; Spanish is helpful but not required.
- Must be able to meet the requirements of applicable federal, state and organizational background checks.
