Eskenazi HealthIndianapolisThe CCBHC Veterans and Criminal Justice Liaison (“Liaison”) is responsible for : (A) ensuring that veterans and certain U.S. Armed Forces members have access to intensive, community-based treatment within the Certified Community Behavioral Health Clinic (CCBHC) as well as continuity of care; (B) supporting clients to minimize contact with law enforcement and the criminal justice system while promoting individual and public safety; and (C) ensuring continuity of care for CCBHC clients involved in the criminal justice system (i.e. through incarceration, parole, probation, juvenile detention, diversion program, arrest, Marion County specialty courts, Marion County Re-Entry Coalition, or any other facet or extension of the criminal justice system). The Liaison provides case management and linkage to social determinants of health, primary care, and other needed resources. The Liaison is responsible for developing and maintaining relationships with the systems that impact veterans and / or those involved in the criminal justice system. The Liaison maintains close collaboration with the CCBHC outpatient treatment teams and continues to actively work with CCBHC clients in the community until the primary CCBHC treatment team can fully engage the client. The Liaison additionally has the important role of tracking and reporting their work with CCBHC clients consistent with CCBHC quality metrics.
Essential Functions and Responsibilities
- Embraces, understands, and operates under the Recovery Model, including AIDET (Acknowledge, Introduce, Duration, Explanation, Thank You) and the spirit of motivational interviewing Regular contact is maintained with the veteran as clinically indicated if ongoing care is required
- For veteran’s, works with the veteran’s psychiatrist or advance practice provider (APP) to ensure delivery of care consistent with the VHA Uniform Mental Health Services Handbook, including reconciliation of each veteran’s psychiatric medications on a regular basis
- Routinely works with professionals in the criminal justice system including incarceration, parole, probation, diversion program, juvenile detention, arrest, Marion County specialty courts, Marion County Re-Entry Coalition, or any other facet or extension of the criminal justice system to ensuring timely and appropriate coordination of care
- Coordinates and helps CCBHC clients navigate release from incarceration to outpatient or other needed treatment to minimize time between release and treatment follow-up
- Helps the primary outpatient team coordinate and develop the client’s treatment plan incorporating input from the client, client’s family, and any representative with decision-making capacity
- Ensures the treatment plan is monitored, documented and revised appropriately, including tracking progress in the care delivered, the outcomes achieved, and the goals attained
- Responsible for ensuring communication between the Liaison, the principal therapist, and the client (and the client’s authorized surrogate or family or friends when appropriate and when clients with adequate decision-making capacity consent) about the treatment plan, and for addressing any of the client’s problems or concerns about their care
- For veterans, ensures the treatment plan reflects the veteran’s goals and preferences for care and that the veteran (or surrogate with decision-making capacity) verbally consents to the treatment plan in accordance with VHA Handbook 1004.1, Informed Consent for Clinical Treatments and Procedures
- Routinely works with professionals in the client’s community, including the VHA, probation, parole, etc.
- Develop, revises, or monitors crisis prevention and safety plans to reflect current CCBHC client status and needs
- Monitors and documents CCBHC client progress including reasonable attempts to contact CCBHC clients post-release from hospitalization and / or incarceration
- Actively works with CCBHC outpatient treatment teams to coordinate transition of CCBHC client to community living post-arrest, incarceration, or hospitalization, including attending relevant team staffings and participation in the Acute Services morning call
- Learns and practices providing case management including working with family, significant others and community supports to assist CCBHC clients transition between incarceration or hospitalization and outpatient settings
- Meets Sandra Eskenazi Mental Health Center requirements for continuing education in accordance with best practice, CCBHC, Joint Commission, and other regulatory standards
- May provide transportation in company provided vehicle and / or personal vehicle as directed by treatment team and guided by medical necessity; maintains a copy of Eskenazi Health’s vehicle-related accident reporting procedures in vehicle
