Job Summary :
Seeking a professional to conduct assessments and develop individualized treatment plans for patients. The role involves implementing therapeutic interventions, providing adaptive equipment recommendations, and collaborating with other healthcare professionals to optimize patient outcomes.
Key Responsibilities :
Conduct assessments to evaluate patients' physical, cognitive, and emotional abilities.
Develop individualized treatment plans based on assessment findings and patients' goals.
Implement therapeutic interventions to improve patients' functional abilities and independence in daily activities.
Provide adaptive equipment recommendations and training to enhance patients' ability to perform tasks.
Collaborate with other healthcare professionals, such as physicians, nurses, and physical therapists, to coordinate care and optimize patient outcomes.
Educate patients and their families about injury prevention, ergonomic principles, and strategies for managing chronic conditions.
Document patient progress, treatment plans, and outcomes accurately and timely in electronic medical records.
Stay current with evidence-based practices and participate in continuing education to enhance clinical skills and knowledge.
Participate in interdisciplinary team meetings to discuss patient progress, discharge planning, and care coordination.
Adhere to ethical and professional standards of practice and maintain licensure and certification requirements.
