Contracted at $80.00 per day plus monthly living expenses.
Schedule : 24 hours per day with scheduled days off for Respite.
Summary : A Structured Family caregiving householder is a contracted individual who agrees to living arrangements in which an individual(s) lives in your private home and receives necessary support services based on their individualized needs outlines in the Person-Centered Individualized Support Plan (PCISP). As a householder you are required to ensure the safety and wellbeing of the individual(s) living in your home in conjunction with Easterseals Arc. This program provides a family like environment that integrates individual(s) served into a family that supports them and builds a deeper connection outside of the typical staffing arrangement. As a householder, you will respond to the needs of the individual(s) served, collaborate with case managers, families, physicians, Easterseals Arc’s staff, and any other involved support team member the individual may have.
Duties and Responsibilities (include but not limited to) :
- Enter daily notes for each consumer in their care. The note should document any progress on goals, medical / health concerns, behavior concerns or any other unusual event.
- Complete daily census for all participant in their home.
- Notify the agency of any unusual incidents immediately.
- Document and administer medication daily per the individual’s needs.
- Individualize tracking based on the needs of the individual in the home.
- Complete safety drills monthly.
- Complete expense ledger quarterly detailing housing expenses (i.e. rent, utilities, groceries).
- Participate in meetings according to the individual(s) need.
- Run all necessary appointments for the individual(s) in the home.
- Engage in all required meetings for the individual(s) in the home.
- Ensure accurate and proper documentation of services; ensures that services adhere to regulations.
- Maintain a clean and livable home for the individual.
- Ensure the individual(s) in the home are working towards independence and inclusion.
- Provide transportation to the participant when needed.
- Maintain a clean criminal background at all times.
- Ensure the health and safety of the individual(s) in the home at all times.
- Ability and willingness to support individuals with challenging behavior and / or medical concerns.
- Comply with all standards of the program. Maintain a valid driver’s license and an insured vehicle at all times.
- Maintain liability insurance.
