We are seeking a dedicated Permanent Primary Care Physician specializing in Family Medicine or Internal Medicine to join our esteemed healthcare team. This role involves providing comprehensive, patient-centered care to a diverse patient population, ensuring the highest standards of medical excellence. The successful candidate will work collaboratively within a multidisciplinary team to enhance patient outcomes and contribute to the overall well-being of the community we serve.
Responsibilities
- Provide comprehensive and continuous medical care to patients across all age groups, ensuring high-quality treatment and preventive measures.
- Conduct thorough patient examinations, diagnose medical conditions, and develop individualized treatment plans.
- Collaborate with multidisciplinary healthcare teams to coordinate patient care and optimize health outcomes.
- Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with healthcare regulations.
- Educate patients and their families on health maintenance, disease prevention, and treatment options.
- Participate in ongoing professional development and stay current with advancements in family and internal medicine.
- Contribute to the improvement of clinical practices and protocols within the healthcare facility.
JOB CAPABILITIES
Value Based Care Model
Physician : As the ‘quarterback’ or ‘captain’ of the team, physician will deliver holistic care to patients
Working in a coordinated manner with a care team comprised of a clinical pharmacist, nurse, health coach, behavioral health specialist, and medical assistant.
Clinical experience in family medicine, internal medicine, or ambulatory care setting is preferred.
MD / DO Board Certified / Board Eligible (Prefers Doc earlier in their career)
Schedule 8-5 : 30 M-F (have been flexible with 4x10hr shifts. Last Appt. ends at 4 : 40p
EMR : NextGen
NO Call
Ea. visit 30 minutes Avg. -Annual Wellness visits 1hr 12-15 patients a day / Complex patients seen 6-8x’s a year
Patient Demographic : Mostly Seniors. Some Peds.
Independent Primary care center not affiliated with the hospital
Uses With Anthem PicassoMD for referrals
Partner w / LabCorp
Urgent Care Center next door
Qualifications
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
Board certification in Family Medicine or Internal Medicine.
Valid and unrestricted medical license to practice in the relevant state.
Proven experience in primary care settings, with a focus on comprehensive patient care.
Strong diagnostic and clinical skills with a commitment to evidence-based practice.
Excellent communication and interpersonal skills to foster patient relationships.
Dedication to continuous professional development and staying current with medical advancements.
