Compensation: $90.00 to $95.00 per hour.

Center Point DAAC's mission is to provide comprehensive social, educational, vocational, medical, psychological, housing and rehabilitation services to combat social problems such as substance abuse, poverty, unemployment and homelessness. Center Point DAAC offers rehabilitation and treatment services that interrupt the abusive cycles of psychological, social and economic dislocation by providing critical training and support so that individuals and families can claim self-worth and dignity.

Job Summary:
Center Point DAAC is seeking a skilled and compassionate Nurse Practitioner to join our team, providing high-quality patient care, conducting intake and annual physicals, health screenings, and consulting with clients regarding dosing issues. Experience working with clients struggling with addiction, specifically substance abuse and alcohol use disorders, is required. Ideally, the candidate will possess a DEA license to prescribe buprenorphine, enabling them to provide comprehensive care for clients with opioid use disorders.
Essential Duties:Medical Procedures, Documentation, & Reporting:
  • Coordinates with Medical Director on intake and initial physician orders.
  • Performs Medical History and Physical Examinations on all clients at intake.
  • Participates in necessary blood drawings, prepares blood specimens for laboratory and ensures pick-up. Dosing duties as needed may include preparing, dispensing and clean up after daily medication dispensing and evaluate side effects and patient's reactions to medication daily, entering dosing orders and exceptions.
  • Administers PPD's (TB test) and follows up with Public Health for patient's recommended treatment when test results are positive.
  • Performs Health Screenings and PPD's to new hires and annually for CP-DAAC staff.
  • Prescribe psychiatric and medication-assisted treatment medications, electronically send them to pharmacies and follow up with any pharmacy inquire.
  • Responsible for ordering all medical and examination supplies.
  • Performs in person Mental Health assessment, crisis intervention and coordinates with Medical Director on mental health clients.
  • Ensures correspondence and lab reports are signed by Medical Director and filed in patient charts. Performs annual History and Physical Report on appropriate clients.
  • Performs follow up and provides recommendations and referrals on identified medical problems or positive findings from History and Physical report and other health related client visits.
  • Serves as liaison with Doctors, Hospitals, Mental Health Care Workers, and other professionals regarding patients.
  • Ensures compliance of Title IX Regulations and Federal Law regarding Intake requirements. Demonstrates positional appropriate knowledge of SMARTCARE database and other computer applications as required.
  • Participate in Medical & Psychiatric Program Development across all of CP DAAC's programs.
  • Direct liaison to Medical Director.
  • Provide training, support and supervision for dispensing staff.
  • Coordinates with Sonoma County Detention Facilities to administer medication to incarcerated patients. Coordinates with hospitals and inpatient programs regarding patient medication.
  • Perform Impaired Client Assessment for those clients that appear under the influence.
  • Participates in the updating of the Dispensing Manual and UOM.

Teamwork & Communication:
  • Direct contact with Medical Director and REAP team.
  • Treats clients, prospective clients, co-workers, and visitors in a courteous, respectful, and professional manner.
  • Participates in weekly staff meeting.
  • Positively represents the program/organization in the community.
  • Performs other duties within clinic as deemed necessary and appropriate by the Program Director.
  • Assures all license and certification requirements are current.

Administration Support:
  • Completes ADP Health Screening Report (including TB test) for all new hires in a timely manner. Reviews PPD (TB test) results submitted by new hires that had test completed elsewhere within (maximum) 60 days prior to employment.
  • Administers PPD for CP DAAC new hires in a timely manner.
  • Administers annual PPD for CP DAAC employees.
  • Forwards all Screening Reports and test results to Human Resources in a timely manner.
  • Notifies HR and refers any staff with positive PPD test results to appropriate Primary Care provider or agency for clearance.

Qualifications:
  • Current license as a Nurse Practitioner with Psychiatric training and experience.
  • Master's level education or higher.
  • Education and/or experience demonstrating basic counseling skills and knowledge of addiction and treatment.
  • Possess good written and verbal communication skills.
  • Accurate mathematical calculation skills.
  • Ability to maintain productive work relationships with patients, employees, management, external agencies and public.
  • Ability to maintain appropriate boundaries with clients and staff as defined by DAAC's Code of Conduct.
  • Ability to use effective judgment in therapeutic and crisis situations.
  • Ability to recognize and manage personal bias.
  • Ability to solve problems and resolve conflicts effectively.
  • Ability to maintain own psychological well-being.
  • Maintains current CPR and First Aid certification.
  • Maintains current license/certification and supporting CEU's as required.
  • Possesses a valid California Driver's License.
  • Unencumbered California State License for necessary functions.
