Innovative Medical Group is a leading Medical Group that provides high-quality, patient-centered care to the underserved post-acute patient population. We seek a motivated and compassionate mid-level provider to join our dynamic team. If you are passionate about endocrinology and want to make a difference in patients' lives, we encourage you to apply. We are interested in candidates who are capable of rounding autonomously within a team, especially those with 1 year of prior acute/hospital experience.

Job Summary

As an Outpatient Endocrinology Nurse Practitioner/Physician Assistant, you will play a critical role in the assessment, treatment, and management of patients with endocrine and metabolic disorders, including diabetes, thyroid disease, adrenal disorders, and other hormonal conditions. You will work collaboratively with our multidisciplinary team to provide comprehensive endocrinology services to our outpatient population within the Riverside County area. Your expertise and dedication will be instrumental in promoting optimal disease management, preventing complications, and improving patients' quality of life.

Key Responsibilities

  • Patient Assessment: Perform comprehensive endocrine assessments, including history, physical exams, lab interpretation, and risk stratification for diabetes and hormonal disorders.
  • Treatment Planning: Develop individualized treatment plans based on current clinical guidelines, evidence-based practices, and patient-centered goals.
  • Chronic Disease Management: Manage conditions such as diabetes mellitus, thyroid disorders, osteoporosis, adrenal and pituitary conditions, and metabolic syndrome.
  • Patient Education: Educate patients and families on disease processes, lifestyle modifications, medication adherence, and self-management strategies.
  • Collaborative Care: Work closely with physicians, dietitians, nurses, and other healthcare professionals to coordinate care and optimize patient outcomes.
  • Procedures: Perform and interpret point-of-care tests (e.g., glucose monitoring, A1C, thyroid ultrasound if trained), and adjust medications accordingly.
  • Documentation: Maintain accurate and up-to-date patient records, including assessments, treatment plans, progress notes, and outcomes.
  • Quality Improvement: Participate in initiatives to enhance endocrinology service delivery, patient safety, and overall quality of care.
  • Patient Advocacy: Advocate for patients’ needs, ensuring they receive comprehensive and compassionate care tailored to their condition.
Qualifications

  • Education: Master’s degree in Nursing with certification as a Nurse Practitioner (NP). Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) preferred.
  • Licensure: Current state NP license and DEA registration. Advanced Practice Registered Nurse (APRN) certification.
  • Experience: Minimum of 1 year of clinical experience as a Nurse Practitioner, preferably with exposure to endocrinology, diabetes management, or internal medicine.
  • Specialization: Certification in diabetes education (CDE) or endocrinology-focused training is preferred but not required.
  • Communication Skills: Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
  • Team Player: Ability to work collaboratively in a multidisciplinary team and contribute to a positive and supportive work environment.
  • Compassion: A strong commitment to providing compassionate and patient-centered care.
  • Problem-Solving: Strong critical thinking and clinical decision-making skills to manage complex endocrine conditions.
