Description
The Unit Coordinator reports to the Practice Administrator and will be embedded within the assigned Care Teams. S / he will work with assigned providers and nurses and other members of the interdisciplinary care team external providers and directly with patients and families to process and coordinate a variety of administrative tasks. The Unit Coordinator plays an essential role with supporting providers nurses and patients and must display excellent written and verbal communication and outstanding customer service. This includes assisting the clinic with creating a welcoming and supportive environment which fosters and promotes patient centered care.
PRINCIPLE DUTIES AND RESPONSIBILITES
- Assist with coordinating patient care within and across departments and externally.
- Answers phone calls or electronic inquiries using good judgment; respond to questions or requests by patients providers and other personnel in a timely manner.
- Aids in the referral management processes. Act as a resource for providers and care team members. Assist with obtaining or following up on urgent or emergent prior authorizations as instructed by providers or nurses within the care team.
- Follow up with patients that had an emergency room visit or was admitted or discharge and requires a follow up appointment with their provider or a nurse.
- Supports patient reengagement outreach activities through direct phone calls and various communication applications and registry reports.
- Process normal lab results pharmacy authorizations durable medical equipment transportation and other form requests; works with providers nurses and other care team members to address escalated patient concerns and outstanding requests (forms consult notes imaging reports prescription refill requests and external agency requests) to ensure timely follow up.
- Process incoming and outgoing mail and patient forms this includes consistently checking fax machines regularly sorting and documenting receipt of forms and degree of urgency including faxing completed documents in a timely manner.
- Track patients requests and work with providers to complete patients inquires in a timely manner.
- Support onboarding and off boarding of providers submitting requests for computer and phones; ordering business cards and supplies; perform clerical duties including but not limited to ordering and stocking nonmedical supplies checking department and provider officers to confirm copiers printers and fax machines have paper; trouble shooting issues with phones computers; call facilities to address maintenance and cleaning requests.
- Perform other administrative tasks as assigned. This includes covering checkin checkout and other patient registration managed care or call center duties as needed based on business needs.
