Description
- MUST RESIDE IN ALBUQUERQUE, NM / NO LODGING PROVIDED
JOB PURPOSE : The case manager is responsible for using sound professional judgment and best practices to ensure quality case management services are provided to the service population especially in crisis situations while maintaining compliance with established state and federal standards. The Case Manager is responsible for intake, coordinated assessment, individualize housing plan, and making appropriate community referrals for additional services. The Case Manager maintains appropriate contact with the client and ensure quality services is provided and that the individualize housing plan is completed timely.
Qualifications
ESSENTIAL JOB RESPONSIBILITIES :
ESSENTIAL QUALIFICATIONS :
EDUCATION : Bachelor's in Social Work, Psychology, Criminal Justice, Sociology or related field of study.
EXPERIENCE : 1+ year related experience and / or training including working in Crisis intervention.
Proficiency in Spanish / English (written and spoken language skills), required.
ATTENDANCE : Must maintain regular and acceptable attendance at such level as is determined in the
employer's sole discretion.
LICENSES : Driver's License with a clear record.
VEHICLE : Must have daily use of a vehicle without prior notice.
OTHER : Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself / herself or others.
