Georgina Home Care is seeking a highly motivated and compassionate Care Manager to join our team in Waterbury, CT. As a Care Manager, you will be responsible for ensuring the well-being of our older adult clients in a home care setting. Your major end result will be to provide exceptional social services to our clients, including, mental health support, and long-term care management . You will work closely with our clients and their families to develop personalized care plans that meet their needs and preferences. Your role will be critical in ensuring our clients receive the highest quality of care possible.
