You will be joining St Vincent’s as we launch our exciting new vision which will take the best of St Vincent’s care beyond our hospitals and aged care homes directly into the homes of our communities across the country, making care more personal and accessible. is more than a strategy. Reporting directly to the National Director SVVH, the Program Manager, In-Home Aged Care will be responsible for overseeing and leading the operations and strategic growth for aged care home care services, including Home Care Packages and the Commonwealth Home Support Programme (CHSP), ensuring our strategy becomes a reality for clients across the community. You will provide oversight of all operational, financial, and workforce requirements for the Home Care team, ensuring quality person-centred care is delivered to aged care clients within the community in line with Commonwealth Aged Care Quality and Safety Standards.

This is a 12-month fixed term full-time position (with review for permanency) based in any of our Group Head Offices in Sydney, Brisbane, or Melbourne on a hybrid working arrangement.

Key responsibilities of the role :

  • Lead multidisciplinary teams to deliver high-quality, person-centred care that exceeds National Aged Care Quality and Safety Standards
  • Partner with St Vincent’s Leaders to launch new services and products, pioneering the future of home-based health and aged care
  • Manage day-to-day operations of the Home Care portfolio, including people, processes, systems, and clients
  • Build and mentor a high-performing, purpose-driven workforce with a focus on wellbeing, capability development and a culture of continuous learning
  • Partner with internal and external stakeholders to optimise service expansion and navigate the transition of the new National Support at Home Program
  • Support and work with the Quality and Clinical Governance team to ensure compliance and formal accreditation
  • Cultivate collaborative relationships across the “One St Vincent’s” network and with external government and industry partners

Things that work for us :

  • Tertiary qualifications in nursing, allied health, or another relevant discipline with post graduate qualifications in business or management desirable
  • Proven leadership experience in home and aged care service delivery, including managing multi-disciplinary teams with a deep understanding of current and emerging regulatory requirements
  • Extensive experience with Aged Care Quality and Safety standards and understanding of the new Support at Home Program requirements
  • Demonstrated ability to streamline processes, improve care experiences, and drive service excellence in complex environments
  • A high level of integrity and the capacity to foster a culture of performance and bold experimentation
  • Excellent written and verbal communication skills with a commitment to values-led leadership
  • Successful completion and maintenance of Responsible Person screening checks. This position is considered a ‘Responsible Person’ role, being accountable for day-to-day operations, and having authority, decision-making, influence & responsibility for planning and directing activities within our organisation
