Job description

Whole Person Care for a Stronger Community

Position Primary Purpose:

The Primary Care Provider (PCP) provides direct patient care under the guidance of the Medical Director, including performing histories, physicals, and ordering diagnostic tests and medications. This role involves completing primary care visits, developing and administering treatment plans for patients and chronically ill clients, and participating in clinical care coordination and case management with the integrated primary care behavioral health team. The PCP ensures timely and accurate medical records documentation using the Electronic Health Record (EHR) system and contributes to the continuous improvement of clinical procedures and policy.

Essential Functions:

Provides direct patient care as directed by the Medical Director.

Provides comprehensive primary care services across the lifespan, including preventive care, chronic disease management, gynecologic care, and age- and risk-appropriate health screenings (e.g., cancer, STI, and wellness screenings), ordering appropriate diagnostic tests, medications, and follow up.

Completes an average of 10-15 primary care visits per day and establishes and administers individualized treatment plans for patients.

Participates in clinical care coordination, consultations, and case management with members of the integrated primary care behavioral health team.

Ensures timely and accurate medical records documentation for all patients in accordance with clinic policies.

Contributes to the continuous improvement of procedures, policies, and materials affecting the medical clinic and its patients.

Utilizes the Electronic Health Record (EHR) system effectively to meet or exceed documentation requirements, achieving clinical quality benchmarks required by grant funders and clinic initiatives.

Provides back up on our mobile medical clinic with the ability to own a mobile medical clinic shift if desired.

Collaborates with providers within White Bird Clinic for clinical care coordination, consultations, warm hand-offs, and case management to support integrated patient care.

All other duties as assigned.

Key Competencies:

Proficiency in providing direct patient care under the guidance of the Medical Director.

Able and willing to work with complex medical patients with high needs under social determinants of health, facilitating connections for that client to other needed services.

Experience treating people with substance use disorders and Severe and Persistent Mental Illness (SPMI).

Ability to perform comprehensive patient assessments, including histories and physical examinations.

Expertise in ordering and interpreting diagnostic tests and prescribing medications.

Skill in developing and administering individualized treatment plans for a diverse patient population.

Commitment to timely and accurate documentation of medical records in accordance with clinic policies.

Continuous improvement mindset, contributing to the enhancement of clinic procedures, policies, and materials.

Effective use of Electronic Health Record (EHR) systems to meet documentation requirements and achieve clinical quality benchmarks.

Strong understanding of national standards and best practices in primary care and behavioral health integration.

Minimum Requirements and Education:

Current valid and unrestricted Nurse Practitioner (NP) license with prescriptive privileges.

Minimum of two (2) years of experience as an independent practitioner in a primary care setting.

Proven ability to work effectively with minimal supervision.

Strong computer skills, including proficiency in Outlook, Word, Excel, and Electronic Health Record (EHR) systems.

Experience working with marginalized populations.

Comprehensive understanding of healthcare regulations and best practices.

Certification by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).

Excellent interpersonal and communication skills.

Preferred Qualifications:

Current valid and unrestricted Family Nurse Practitioner (FNP) license with prescriptive privileges.

Proficiency in both English and Spanish.

Compensation Details:

This is a full-time, non-union, salaried exempt position with target base wage range that is DOE (Depending on Experience), paid out on a bi-weekly basis.

Employee Medical Insurance with an option 100% paid by the employer.

Employee Dental Insurance is 100% paid by the employer.

Employee Vision Insurance is 100% paid by the employer.

$1250 annual Health Reimbursement Account prorated from start month.

$30 per month wellness reimbursement for qualifying activities or free membership to the Eugene YMCA.

401(k) option with up to a 3% match.

Paid Time Off (Accrual up to 25 days per year and increases with tenure).

Paid Holidays (9) plus floating holiday = 10 paid holidays.

A two (2) month paid sabbatical for employees.
