Hummingbird Home Care, LLC is seeking a professional and compassionate Home Health Care Coordinator to support the growth and quality of our home care services. The ideal candidate will have experience in client assessments, care coordination, and community outreach and marketing within the healthcare or home care industry. This role is responsible for developing client relationships, conducting in-home assessments, and ensuring seamless communication between clients, families, and caregivers.

Essential Duties and Responsibilities :

• Conduct thorough client assessments to determine individual care needs and create customized care plans.
• Coordinate and oversee the matching of caregivers to clients based on skills, experience, and compatibility.
• Serve as the primary point of contact for new and existing clients to ensure quality service, delivery and satisfaction.
• Develop and maintain relationships with referral sources, including hospitals, rehabilitation centers, senior communities, and healthcare providers.
• Participate in community outreach and marketing activities, representing the agency at local events and professional meetings.
• Perform regular follow-ups and quality assurance visits to monitor service effectiveness and client well-being.
• Collaborate with scheduling and operations teams to ensure continuity of care and efficient communication.
• Maintain accurate documentation and records in compliance with agency policies and regulatory requirements.
