Health Care Occupational & Case Manager

Immediate need for a talented Health Care Occupational & Case Manager. This is a 12+ months contract opportunity with long-term potential and is located in Baltimore, MD (Hybrid). Please review the job description below and contact me ASAP if you are interested.

Job ID:26-27631 Pay Range: $37 - $38/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

Assessing patients' medical, psychological, and social needs.
Developing individualized care plans in collaboration with healthcare providers and patients.
Coordinating healthcare services, including medical treatments, therapies, and consultations.
Monitoring patients' progress and adjusting care plans as needed.
Providing education and support to patients and their families.
Advocating for patients' needs and rights within the healthcare system.
Communicating with healthcare providers, insurance companies, and other stakeholders to ensure coordinated care.
Evaluating the effectiveness of care plans and making recommendations for improvement.
Documenting patient assessments, care plans, and interactions.
Keeping abreast of healthcare regulations, policies, and best practices related to case management.
Health Care Occupational and Case Managers should have a background in nursing, social work, or a related field, along with experience in case management.
They should have strong communication, organizational, and problem-solving skills.
A compassionate and empathetic approach to patient care is essential.
Depending on the specific role, certification as a Case Manager (CCM) or in a related field may be required or preferred.
Schedule: 7am-3:30pm.
Will report to location on Thursdays or as needed.
On-call weekly rotation schedules every 4 weeks.
Will be on call 24-hours for work-related injury, illness, or exposure reporting.

Key Requirements and Technology Experience:

Strong customer service, assessment & adaptability skills
Critical care experience, occupational health, or case management experience
Current BSL (Basic Life Support) Certification.
Registered Nurse Degree: Bachelor of Science in Nursing or Master of Science Degree preferred
