Job Description Be a part of a world-class academic healthcare system, UChicago Medicine , as a Pediatric Care Coordination Manager for the Inpatient / Outpatient ER.In this role the Care Coordination Manager will provide management and supervision to Social Workers, Case Managers, Patient Navigators, and Registry Social Worker staff and to assist the director in the day-to-day functioning of the department through scheduling, quality assurance, coverage, problem solving, recruitment, evaluation of staff competency and performance and maintenance of department budget. Works closely with the Director of Care Coordination.Essential Job Functions : Provides clinical supervision and oversight of staff Social Workers, Case Managers, Patient Care Navigators, and Registry Social Workers in pediatric and adult hospitals, ensuring quality of service on a 24 hour-basis.Ensures comprehensive care coordination assessment occurs for all patients within 24 hours of admission, including documentation of EDD, anticipated discharge disposition and care progression barriers in Epic.Supports MDRs by role modeling best practices for team members, supervising MDRs across sub-teams, and escalating barriers (incl. need for additional education and changes to MDR flow).Reviews outlier patients and supports barrier resolution, including participation in complex case meetings for assigned patients.Ensures that supportive patient-centered psychosocial services are delivered to patients, their families, and to members of the medical center's community in a timely manner.Supports in emergency and crisis situations and is an active, experienced problem-solver.Assesses urgent or crisis social work needs, maintains availability for patient-initiated requests, conducts routine visits to patients in designated areas, provides social work as requested / needed. May carry a clinical load.Supervises meetings with patients, family members and staff upon request for confidential support and supervision.Contributes to the development and continuous improvement of the Department of Care Coordination.Effectively communicates the mission and philosophy of the medical center to staff, patients, visitors, medical staff, the community and outside agencies.Participates in staff committees and task forces to support excellence in care.Serves on various hospital committees as directed by the Director of Care Coordination.Supports the Director in managing the department within the framework of the annual budget.Demonstrates respect / appreciation for persons served by the Department, regardless of background, race, age, sexual orientation, gender, religion or disability.Maintains awareness and knowledge of current clinical issues or advances relevant to departmental decision-making and functioning.Participates in regular continuing education and regional / national professional meetings, representing the department.Understands changes in health care delivery and functions effectively within health care systems.Promotes and has knowledge of and utilizes appropriate patient care protocol relating to the physical, psychosocial and developmental needs of patients.Establishes relationships with other health care professionals and community groups and coordinates the services of the department relating to other specified agencies.QUALIFICATIONS : Requires a Master of Social Work from a graduate school of social work accredited by the Council on Social Work Education with active LSW (LCSW preferred); or a bachelor’s degree in nursing with an active Registered Nurse (RN) license.Requires Five (5) years of experience in a Medical Social Work setting or an equivalent 1-3 years of experience in a leadership role.Ability to take initiative in projects and tasks, ability to use independent judgment / insight to solve problems, and ability to show evidence of an adherence to principles of social work practice.LICENSES / CERTIFICATIONSCurrent L.C.S.W or RN from the State of Illinois.Position DetailsJob Type / FTE : Full TimeShift : Days / Monday-FridayWork Location : Comer Children’s HospitalUnit / Department : Transition and ContinuityCBA Code : Non-UnionWhy Join Us We’ve been at the forefront of medicine since 1899.
