Position Overview
We are seeking a dedicated and compassionate Case Manager to join our outpatient mental health services team. The Case Manager will play a pivotal role in supporting clients with their mental health needs, focusing on key areas such as daily living skills, symptom monitoring, emotional regulation, crisis coping, and interpersonal skills. The ideal candidate will be committed to improving the quality of life for our clients through comprehensive assessment, service coordination, and continuous support.
Key Responsibilities
Assessment of Client's Medical, Social, and Functional Status :
- Conduct comprehensive assessments to identify client strengths and needs.
- Develop individualized care plans based on assessment findings.
Arranging for Service Delivery :
Facilitating Access to Needed Services :
Facilitating Access, Quality, and Delivery of Necessary Services :
Preparation and Maintenance of Case Record Documentation :

Working Conditions
The Case Manager will work in an office setting and will also conduct home visits and accompany clients to appointments as necessary. The role may require occasional evening or weekend hours to accommodate clients' needs.
