Job descriptionCoordinate with clients, their legal guardians, or custodial parents to select appropriate service providers. Ensure clients have access to necessary services by facilitating appointments and referrals. Explain the importance and relevance of services to clients in relation to their condition. Advocate for clients to ensure they receive the services they require. Monitor the quality and effectiveness of services provided to clients. Address any barriers to service delivery and work to resolve them promptly. Maintain accurate and up-to-date case records, including care plans, forms, reports, and narratives. Ensure documentation complies with organizational and regulatory standards. Assist clients in developing routines and strategies to manage daily tasks effectively. Provide guidance on personal hygiene, meal preparation, and household management. Educate clients on identifying and tracking symptoms and medication side effects. Collaborate with clients to implement strategies for managing and reporting changes in their condition. Teach clients techniques for managing emotions and stress. Support clients in applying these techniques in their daily lives to enhance emotional stability. Provide clients with tools and strategies to manage crises effectively. Work with clients to develop personalized crisis plans. Facilitate activities and exercises to improve social skills and communication. Encourage clients to build and maintain healthy relationships. Possess a bachelor of arts or science degree in a behavioral science; be a certified alcohol and drug counselor who has a bachelor of arts or science degree; have provided targeted case management services to a recipient any time from April 1, 2014, to the effective date of this administrative regulation; or have supervised the provision of targeted case management services to a recipient any time from April 1, 2014, to the effective date of this administrative regulation; Relevant certification or licensure as required by state regulations. Have at least one year of experience in a human services setting working directly with adults or youth. Successfully complete a department approved targeted case management training within six (6) months of employment as a targeted case manager; and Successfully complete department approved continuing education requirements every three (3) years thereafter. Complete Kentucky department approved training requirements for a Community Support Associate within 6 months of hire. Strong understanding of mental health conditions and treatment options. Excellent communication, organizational, and interpersonal skills. Ability to work collaboratively with a multidisciplinary team. Proficiency in case management software and other relevant technology. The ideal candidate will be committed to improving the quality of life for our clients through comprehensive assessment, service coordination, and continuous support.
Key Responsibilities
